This Privacy Policy governs the manner in which EPIC4 Specialty Partners and its affiliates (individually and collectively, “EPIC4 Specialty Partners,” “we,” “us,” or “our”) collects, uses, maintains and discloses information collected from users (each, a "User") of the EPIC4 Specialty Partners website ("Site").
By accessing or using the Site, by accepting the Terms of Use, or by entering into any other agreement with EPIC4 Specialty Partners that governs access to or use of the Site, you agree on behalf of yourself and any organization or company that you represent (together, “you”) that you have read and understand this Privacy Policy. If you do not agree with this Privacy Policy, do not access or use the Site.
This Privacy Policy is incorporated into and made a part of our Terms of Use and any other agreement that references this Privacy Policy or that governs access to or use of this Site.

Information You Submit
Personal identification information. We may collect personal identification information from Users in a variety of ways, including, but not limited to, when Users visit our Site, register on the Site, fill out a form, respond to a survey, and in connection with other activities, services, features or resources we make available on our Site. Users may be asked for, as appropriate, name, email address, mailing address, phone number. Users can visit the Site without supplying personally identification information, except that it may prevent them from engaging in certain Site related activities.
Job Applications. If you submit a resume, job application, or related materials or other information to us (where those options are available to you), we may use that information to evaluate your qualifications and consider or respond to your inquiry or application. Your submission of a resume, job application, or related information does not in any way require EPIC4 Specialty Partners to review that information or consider you for employment. To view career opportunities at EPIC4 Specialty Partners or to submit a job application (where those options are made available), you may be redirected to an online career portal operated by a third party. We encourage you to review any terms and privacy policies posted on that portal (if applicable).
Contact Form, Quotes, Requests for Support or Information. To contact us for more information, quotes for our applicable products and services, or to request support via the Site, you will need to provide your contact information (including, for example, your name, email address, telephone number, and physical address) as applicable, details of the products or services you are interested in, the company you work for or practice you work under, and the subject matter in which you are interested.
Marketing Communications. If you sign up or otherwise opt in to receive newsletters or other communications from us, you will need to provide your email address. We will use this information to send you Epic4 Specialty Partner updates and promotional and other electronic communications. You may opt out of receiving updates and other emails from us at any time by following the unsubscribe instructions in the applicable email. We may use third-party email provides to deliver these communications to you.
Other Information Collected
As Users navigate and use the Site, certain information can be passively collected—that is, gathered without the User actively providing the information or being concurrently made aware of the collection of information—using various technologies. We passively collect a variety of types of information in a variety of ways. We may collect non-personal identification information about Users whenever they interact with our Site. Non-personal identification information may include the browser name, the type of computer and technical information about Users means of connection to our Site, such as the operating system and the Internet service providers utilized and other similar information.
IP Addresses and Related Data. The servers used to operate and provide the Site may collect data pertaining to Users and the equipment, software, and communication methods Users use to access the Internet and the Site, including Internet protocol (“IP”) addresses assigned to the computers and other devices from where Users access the Internet, Users’ Internet service provider (ISP), device ID numbers and unique identifiers, Users’ media access control (MAC) address, Users’ operating system, Users’ computer screen resolution, Users’ web browser type, the pages Users access on the Site, the websites Users access before and after visiting the Site, the length of time Users spend on the Site, date and time stamps, clickstream data, Users’ approximate geographic location, performance statistics, and usage data. We may use this information to administer the Site and its servers, to generate statistical information, to monitor and analyze Site traffic and usage patterns, to monitor and help prevent fraud, to investigate complaints and violations of our policies, and to improve the Site.
We may combine this information with other information collected through the Site (including personal information) and information obtained from third parties for security reasons and to protect our rights or the rights of others. The suppliers that we use to provide the Site may collect information about Users’ visits to the Site and other websites. Some of this information may be collected using cookies and similar tracking technologies as explained below under “Web browser cookies” section below.
Web browser cookies. Our Site may use "cookies" to enhance User experience. User's web browser places cookies on their hard drive for record-keeping purposes and sometimes to track information about them. We use cookies to help us improve User access to the Site (for example, by remembering User contact and other information when Users access or use the Site), to identify repeat visitors to the Site, to track how Users access and use the Site, to learn when and how users visit the Site, to learn how popular Site pages are and aren’t, to learn which search terms are used to find the Site, to learn which websites direct Users to the Site, to help display certain information on the Site, and to improve Users’ enjoyment of the Site.

A cookie can either be a “session” cookie or a “persistent” cookie. Session cookies exist only for so long as you are visiting the applicable website. Each time you visit a website that has implemented a persistent cookie, that cookie will remain active until its predetermined expiration date. The Site may also use the following types of tracking technologies: web beacons (also called clear GIFs), flash cookies, and pixels (also called pixel tags). Web beacons are tiny graphics with unique identifiers that functions similar to how cookies function but, in contrast to cookies, web beacons are embedded invisibly on websites. Flash cookies collect and store information about your use of a website and are commonly used for advertisements and videos. Pixel tags can be placed on websites or within emails to track your interactions with those websites and when emails are opened.
Social Media. The Site may allow Users to connect to and share information with social media platforms and we may be required to implement cookies, plug-ins, and APIs provided by those social media platforms in order to facilitate those communications and features. We may share information that Users provide us or that we may collect about a Users use of the Site with those platforms and that information will be subject to their privacy policies. We encourage Users to review the privacy policy of any social media platform that Users use in connection with the Site. In addition, by choosing to use any third-party social media platform or choosing to share content or communications with any social media platform, Users allow us to share information with the designated social media platform. We cannot control any policies or terms of such third party platform. As a result, we cannot be responsible for any use or disclosure of your information or content by third-party platforms, which you use at your own risk.
We are active on social media. Users may have the opportunity to comment on those social media platforms regarding Epic4 Specialty Partners and the products we offer and/or to submit or upload related photographs and other materials. We reserve the right to post on the Site, other websites, and social media pages any comments, photographs, or content that you post on our social media pages or provide to us through social media.
How We Respond to Do-Not-Track Signals. Due to the automatic collection of data using cookies as described above, we do not honor “do not track” requests. For this reason, a User’s selection of the “Do Not Track” option provided by a User’s browser may not have any effect on our collection of information using cookies and other tracking technologies.
Embedded Content. The Site incorporates content, including feeds, scripts embedded in the Site’s code, and visible content (e.g., videos), provided by third parties. Information From Other Sources. We may obtain data about Site Users from various third-party companies and public sources, whether offline or online (such as publicly available social media profiles), and we may combine that data with other information collected through the Site (including, without limitation, personal information). This enhances our existing information about our Users and customers and improves our ability to contact you and our marketing’s relevancy.

Your Rights and Choices

The above rights are only exercisable by you where the CCPA grants you the right being exerted and where no exception or exemption under the CCPA applies. You may also have a registered agent (if and only as permitted under applicable law) that you authorize to act on your behalf. If you have a registered agent act on your behalf, we have the right to authenticate such agent’s authority to act.
We do not sell your personal information to third parties, nor do we share your information for purposes of cross-contextual behavioral advertising as defined under the CCPA. In the event we begin selling personal information (1) this Notice will be updated to reflect that fact and you will be notified of such update and (2) if required under the CCPA, you will be provided the ability to opt out of the sale of your personal information or the sharing of your personal information for cross-contextual behavioral advertising purposes (as may be applicable).

Only you or a registered agent (if and only as permitted under applicable law) that you authorize to act on your behalf, may make a verifiable consumer request related to your personal information. If you have a registered agent act on your behalf, we have the right to authenticate such agent’s authority to act. You may only make a verifiable consumer request for access or data portability twice within a 12-month period. Without limiting the foregoing, the verifiable consumer request must:
•Provide sufficient information that allows us to reasonably verify you are the person about whom we collected personal information or an authorized representative.
•Describe your request with sufficient detail that allows us to properly understand, evaluate, and respond to it.
We cannot respond to your request or provide you with personal information if we cannot verify your identity or authority to make the request and confirm the personal information relates to you. Making a verifiable consumer request does not require you to create an account with us. Response Timing and Format. We endeavor to respond to a verifiable consumer request within 45 days of its receipt, and in any event, we will respond to you within the timeframe required under applicable law. We do not charge a fee to process or respond to your verifiable consumer request unless it is excessive, repetitive, or manifestly unfounded. If we determine that the request warrants a fee, we will tell you why we made that decision and provide you with a cost estimate before completing your request.
